This form can be submitted at any time during the term. Any form that has missing information or incorrect information will not be processed.
Please Note: The date the form is received by the Registrar's office will be used to determine your eligibility for tuition refund and withdrawal grades per the College's policies.
If the form is received BEFORE 11:59pm PT Friday of the first week of the term, your course will be unregistered, you will not receive a grade and will receive a refund according to your state guidelines.
If the form is received after midnight PT Saturday of week 1 through 11:59pm PT Friday of week 3 you will receive a grade of W and may not be eligible for a refund.
If the form is received after midnight PT Saturday week 3 through end of the term it will not be processed until the end of the term and you will receive grade earned in the course and no refund.
I understand that by submitting this form, I am requesting to be permanently withdrawn from American College of Education. I understand that if I choose to re-enter the College within 365 days, I will go through a re-entry process and will return to the newest program version at that time.
Furthermore, I understand that if I remain withdrawn for a period of 365 days from my last date of attendance, I will need to re-apply to the college and I will fall under the catalog and program version, plus any tuition changes, in effect at that time.
Therefore, I hereby authorize American College of Education to process my status change as indicated above.
