Return from Leave of Absence Request Form
DEADLINE FOR SUBMISSION:
This registration form must be received by the Registrar’s office
than 6 weeks prior to return term start date and
than 11:59pm PT Monday, 1 week prior to the term start for which you are returning. Any form that is not received on time, has missing information or incorrect information, will not be processed.
Example: Term Start date May 21st – Form can be submitted beginning April 9th and must be received by 11:59pm PT May 14th.
I understand that by submitting this form my status will be changed with American College of Education. Therefore, I hereby authorize American College of Education to process my status change as indicated above.
I further acknowledge that by taking a Leave of Absence, it will impact my completion date and my schedule sequence. I understand I may be required to sit out a term(s) at a later date due to course availability.
ATTENTION Educational Leadership students who reside or work in the statue of Illinois:
Due to changes in Illinois State Licensure deadlines, your ability to be licensed in the state of Illinois can be affected by your Leave of Absence. Please contact Registrar@ace.edu Attention Kate Sureck for additional information.
